Poly(ethylene glycol) (PEG) is extensively utilized in the biomedical field
particularly in emerging nanomedicines
owing to its excellent biocompatibility. However
its clinical value is constrained by issues such as immunogenicity-induced production of anti-PEG antibodies and the accelerated blood clearance (ABC) phenomenon. This review focuses on strategies for modifying conventional PEG through linear structural alterations and the development of different topological architectures
including branched
brush-like
and star-shaped structures. By disrupting the continuous ethylene glycol epitopes required for antibody recognition and optimizing the surface conformation and density
these modifications significantly reduce anti-PEG antibody binding and complement activation
thereby circumventing the ABC effect and complement activation-related pseudoallergy (CARPA). The resulting PEG derivatives exhibited superior performance in prolonging the circulation time of nanocarriers
enhancing tumor-targeted accumulation
and improving therapeutic efficacy upon repeated administration.
